Intake and Interview Sheet

Part —IV - Income

Review the taxpayer's responses to Part IV, Income, Form 13614, Intake and Interview Sheet to find out if the taxpayer or spouse received:

  • Investment income (such as interest and dividends)
  • A distribution from and IRA or retirement plan, such as a 401k
  • Social security payments
  • Unemployment payments
  • Income not reported on Form(s) W-2 or Form(s) 1099 (such as from alimony, self-employment, gambling, etc.)

All income is taxable if it is not specifically excluded by law. The Income section of the Intake and Interview Sheet lists possible sources of income. It includes both earned and unearned income.
