2011 Community Based Outlet Program (CBOP) E-Order Form 12815

The IRS e-mailed Form 12815, CBOP Order Form, to active CBOP accounts on 10-03-2011. If you received the e-mail but have not submitted Form 12815, please submit it by November 7.

Any active CBOP participant who has not received Form 12815 should send an email to wi.cbop.administrator@irs.gov and include the following:

• On the Subject Line, type, "Need 2011 Form 12815" and your CBOP Account Number   
• In the body of the e-mail, provide: 

1. The CBOP Account Number
2. Contact's Name
3. Contact's E-mail Address
4. Contact's Daytime Telephone Number
