BSA Prime Lead MemorandumThe BSA Prime Lead Memorandum format outlines the information required by CI to properly evaluate the information item.This is a one page document titled, BSA - Prime Lead Memorandum. Area of Noncompliance and years involved: (Use attachments if needed). The examiner will insert the identified noncompliance and years involved. If attachments are needed, they must be electronically embedded into the PDF document. The examiner completes the next line, which reads: 5. Summary of Available Information/Evidence: (Use attachments if needed); followed by: (Specify the time period of the records reviewed). The next line reads: 6. BSA Examiner: followed by Phone: The examiner electronically signs his/her name and inserts his/her phone number. The BSA manager electronically signs his/her name and inserts his/her phone number on the next line, which reads: 7. BSA Manager followed by Phone: The examiner completes the next line, which reads: 8. Attachments: (List the attachments), by listing any and all attachments referenced in or associated with the memorandum.
