The first column on the left is the Name, the second column from the left is the Executive Name, and the third column from the left is the Contact Name. Immediately following the second row, the third row has three columns. The first column from the left is the Description, the second column from the left is the # Questions, and the third column from the left is the Contact Email. Immediately following the third row, the fourth row has three columns. The first column from the left is the Practice Area, the second column from the left is the Website, and the third column from the left is the Feedback Type. Immediately following the fourth row, the fifth row has one column. This column is the Campaign Project Special Instructions. Another one-column row below the one just described includes the following text, Issue Description:. Three one column rows are immediately below the issue description. Each of the three one column rows contains the text, Identified Issue, with space to describe the identified issue. The case manager or designee inputs domestic tax issues in the identified issues section.
