  • Do not complete this form unless you have received a Letter 566 (T) or (S) and are currently being audited for your electric vehicle credit. 
  • We need to verify your eligibility to claim the credit taken on Form 8936, Qualified Plug-In Electric Drive Motor Vehicle Credit, which you attached to your Form 1040, U.S. Individual Income Tax Return. Our records show the vehicle identification number (VIN) you provided on Form 8936 is either ineligible or invalid. Provide all requested documents to the return address listed at the top of the attached letter.
