What do I do if the amounts on my Form 1099-H don't match my records?

I expected a Form 1099-H. Why didn't I get one?

If you made payments through the program but didn’t receive Form 1099-H, please call us at the phone number on your letter or email us at  wi.hctc.stakehldr.en@irs.gov.

Not all taxpayers who qualify for the Health Coverage Tax Credit will receive a Form 1099-H. Only participants who made payments through the Advanced Monthly Program will receive Form 1099-H.
