Form 15086 may be used to request a facsimile of Form 7249, Offer Acceptance Report. The IRS makes this record available by request for one year after the date of acceptance of an Offer in Compromise.

Form 7249 contains limited information including name, liability amount and offer terms. View a sample Form 7249, Offer Acceptance Report PDF to see the information contained in a public inspection file. Do not use Form 15086 to submit a new offer or inquire about an open offer. See IRS.gov/OIC.
