Complete Form 15106 included with your notice, detach notice stub and return them using the envelope provided. If you prefer, you can call the number at the top of the notice. Please respond within 10 days from the date of your notice.

Why would I want to be excluded from a list of persons contacted?
We’re required by law to keep a list of people we’ve contacted, called third party contacts. In some cases, we are required to provide this to the individual. If you don’t want to be on this list, please call us at the phone number on the top of the first page of your notice.
