The form is titled Application for IRS Individual Taxpayer Identification Number: Reject at the top. A text box is at the very top of the form. Instructions inside the text box state: Use an envelope without a window unless Form 14433 is also used. Below the title, the form has two columns of data.The upper left column has the originating office return address shown as ITIN Office, Austin, Tx 73301-0057The next line has the Reference Number followed by a line on which to enter it.
