Use Form 5310 to request an IRS determination as to the qualified status (under section 401(a) or section 403(a)) of a pension, profit-sharing, or other deferred compensation plan upon plan termination.


Current Revision

Form 5310 must be submitted electronically through Pay.gov.  You can access the most recent revision of the form at Pay.gov.

To submit Form 5310  you must:

  1. Register for an account on Pay.gov.
  2. Enter "5310" in the search box and select Form 5310.
  3. Complete the form.
